Uncaught Error creating field named Title: Unable to parse SPField type.

Hello Kit.

I am using Office 365 SharePoint.

I have followed the instructions but have this issue
1) Firstly I had the issue discussed in the other posts about 'Unable to get an SPField named'. - I went past this by changing the line #329 and 330 from
fieldElements = $('table.ms-formtable td.ms-formlabel') 
to
fieldElements = $('td.ms-formlabel')
2) After getting past the the above issue, I have stumbled across another issue in line 259 as the type is null. This is because of line 127 as the DOM has no such nodeType (8 i.e. for comment). The 'element' DOM is as follows

Apologies if this I am missing something but have you come across this issue?

Thank you very much for the additional information... it is extremely helpful. I will try to help. By any chance have you customized the form using SharePoint Designer? If so, it removes the comments which SPUtility depends on to read the field's type.

You got me! I created the new form using SharePoint designer. Sorry should have remembered this myself. Thank you.
